Loss of license, reinstatement: a case study in how recovery is a team effort

Jake Nichols, PharmD, MBA, was a clinical pharmacy coordinator at Boston Medical Center (BMC) for six months when he began forging prescriptions to feed his opioid addiction. He rationalized that because he was paying for the medications, it was not diversion. Dr. Nichols said he practiced there for more than two years while impaired, and, to his knowledge, never harmed a patient.

While at BMC, “colleagues noticed something was off, but they never accused me of
